Freedom Of Information Act Requests

The following steps are required to be completed when any individual, office, or agency is requesting a copy or copies of Coroner’s Office records:

Request via person, mail, or email a Freedom of Information Act Request Form.
Complete and return the entire FOIA request form to the office.
The Coroner’s Office will respond to your request for records, either by phone or mail, within seven days.
If your request for records is approved by the Coroner’s Office, you will be advised as to when your copies are available, along with the cost of the copies.
If your request for records is denied, you may appeal. Appeals should be addressed to:

Please note that only reports produced by the Coroner’s Office will be released. In accordance with Illinois State Law (Illinois Revised Statutes, Chapter 34, Division 4-7, Coroner’s Fees,
4-7001), the copy fees are as follows:
