Community Health Needs Assessment & Health Plans
Joint 2022 Community Health Needs Assessment
The McLean County Health Department, OSF HealthCare St. Joseph Medical Center, Carle BroMenn Medical Center and Chestnut Family Health Center have collaborated to conduct a joint Community Health Needs Assessment (CHNA). A CHNA is required by provisions of the Patient Protection and Affordable Care Act for the hospitals and by the Illinois Administrative Code Section 600.400 for McLean County Health Department to maintain local health department certification.
The goals of the CHNA was as follows:
- Analyze data collectively
- Prioritize and select the top three health priorities for McLean County
- Generate a joint county community health needs assessment document to be used as a resource for the community
- Work collaboratively with other key community stakeholders to develop implementation plans for each of the top three health priorities, culminating in a joint Community Health Improvement Plan by February 2023 that covers a 3-year period, 2023 to 2025.
The McLean County Community Health Council was formulated in April of 2015 to serve as the advisory body for the collaborative CHNA. The Council now consists of 58 individuals from 39 organizations representing 11 sectors, including healthcare, public health, social services, city/county planning, housing, transportation, education, business/economic development, faith, law enforcement and civic organizations/service clubs. The following three health priorities were selected by the McLean County Community Health Council after analysis of the 2022 McLean County Community Health Needs Assessment:
- Access to Appropriate Care
- Behavioral Health (including mental health and substance use)
- Healthy Eating/Active Living (HEAL)

Joint 2023 to 2025 McLean County Community Health Improvement Plan
The McLean County Health Department, OSF HealthCare St. Joseph Medical Center, Carle BroMenn Medical Center and Chestnut Family Health Center have collaborated to produce a new 3-year Community Health Improvement Plan (CHIP) for 2023 - 2025 for McLean County. In addition to being a best-practice, a CHIP is required by provisions of the Patient Protection and Affordable Care Act for the hospitals and by the Illinois Administrative Code Section 600.400 for the McLean County Health Department to maintain local health department certification.
The joint 2023 - 2025 Community Health Improvement Plan is the second phase of the joint 2022 McLean County Community Health Needs Assessment (CHNA) process. It provides action plans to address the high-level goals for the top three health priorities identified in the 2022 CHNA:
- Access to Care
- Behavioral Health (including mental health and substance use)
- Healthy Eating/Active Living (HEAL)
Each health priority action plan identifies measurable outcome and impact objectives as well as process and outcome indicators. Despite the challenges provided by the COVID-19 pandemic, the three Priority Action Teams (PATs) assigned to work on each of the three priority areas identified in the 2020 - 2022 Community Health Improvement Plan, documented progress toward implementing the specified intervention.

2017 to 2019 Community Health Improvement Plan Progress Reports
In 2017 and 2018, through the efforts of committees assigned to work on each of the priority areas identified in the 2017-2019 Community Health Improvement Plan, considerable progress was made toward implementing the specified intervention strategies and achieving the defined targets.
- In just the first year, community efforts resulted in over half of the targets being achieved or performance improved for several indicators
- Several evidence-based programs were continued or expanded, particularly for the behavioral health initiatives;
- Over $195,000 in grant funding was obtained in 2017 to support improvement plan interventions or related activities; and,
- In 2018, due to strong community partners, "related improvement plan accomplishments" escalated in each health priority.

2012 to 2017 McLean County Community Health Plan & Needs Assessment
Prior to the collaborative CHNA process described above, the McLean County Health Department completed the Illinois Project for Local Assessment of Needs (IPLAN), a community health assessment and planning process that is conducted every five years by local health jurisdictions in Illinois. Based on the Assessment Protocol for Excellence in Public Health (APEX-PH) model, IPLAN is grounded in the core functions of public health and addresses public health practice standards. A component of the IPLAN process is the development of a Community Health Plan (CHP) that identifies strategies to address the top health priorities in our community.
The top three health priorities identified in the McLean County CHP 2012-2017 are as follows:
- Obesity
- Access to Mental Health Services
- Oral Health
