Military Services Members - Death Certificates

Death Certificates for Retired or Active-Duty United States Military Service Members


Death certificates are considered vital records. Only those who have a legal right to the record may obtain the record.


Requirements:

  • Identification of the person applying for the death certificate in the form of a valid photo ID.
  • Proof that the Service Member was Retired or Active-Duty at the time of death
    • DD214
    • Letter from Veteran’s Affair Office
    • Copy of military ID
    • Other

You may request birth or death certificates in 3 different ways: 
By Mail, In Person, or Online.


1) By Mail:

Submit a completed Service Member Death Certificate Application (PDF), a copy of your valid photo ID, proof of the Service Member’s status of RETIRED or ACTIVE-DUTY (see above), and a check or money order for the appropriate fee (see above chart) to the McLean County Health Department.

2) In Person:

Applications are accepted in person from 8:00 am - 4:30 pm, Monday through Friday. Individuals requesting a Birth or Death Certificate must provide a valid photo ID and proof of the Service Member’s status as RETIRED or ACTIVE-DUTY.

3) Online:

ONE FREE DEATH CERTIFICATE – ONLY:

  • Is the date of death before January 1, 1999? Click Here

    • You must upload a copy of your photo ID
    • You must upload proof (see above) of the Service Members’ status as Retiree or Active-Duty
